Use License Forms
Deans may grant a newly-contracted instructor a "use license" for online course materials created by another instructor. Deans are authorized to do so by an "extended use license." This they can obtain in two ways: (a) automatically if the University specifically compensates an instructor for creating a specific course; (b) gratefully if an instructor freely grants the extended use license to the administration. The forms below represent three types of licenses involved. A user fills out a form, saves a copy, and emails the copy to the Office of the Academic & Technical Coordinator of Distance Learning. Informally, any instructor may grant copyrights or permission-to-use to any other instructor.

Used by an dean who ensures compensation to an instructor for creating a specific online course. The instructor, in turn, grants this Extended Use-License to the Administration, whereby it is authorized to extend a Limited Use License of the compensated course materials to another instructor (usually one newly contracted to teach this course).

Used by an instructor who created an online course without specific compensation. It freely grants to the Administration this Extended Use-License whereby deans are authorized to extend a Limited Use License to other instructors. The creator may include stipulations on what can be used and when.
